INSURANCE/PAYMENTS

1. Do I need a referral?

Dr. Hampton is a Specialist.So if you need a referral to see anyone other than your primary care doctor, you will need one to see Dr. Hampton.

If your insurance plan requires a referral, please ensure that the referral is in our office prior to your appointment. Please understand that this is your responsibility. Most HMO plans require referrals.Not having a referral may mean out of pocket expenses to the patient.

2. Do you take My Insurance?

We accept most major insurance. Please take a look at our list of participating insurances (not a complete list). If you don’t find your insurance on this list, call our office regarding plan participation.

3. Payment Methods

All payments are due at the time of service.

4. Do I have a co-pay?

This will depend on your insurance policy.  The co-pay amount is listed on most insurance cards.   Co-Pays will be collected at the time of your visit.
