INSURANCE/PAYMENTS

  1. Do I need a referral?

  2. Dr. Hampton is a Specialist.So if you need a referral to see anyone other than your primary care doctor, you will need one to see Dr. Hampton.

    If your insurance plan requires a referral, please ensure that the referral is in our office prior to your appointment. Please understand that this is your responsibility. Most HMO plans require referrals.Not having a referral may mean out of pocket expenses to the patient.

  1. Do you take My Insurance?

  2. We accept most major insurance. Please take a look at our list of participating insurances (not a complete list). If you don’t find your insurance on this list, call our office regarding plan participation.

  1. Do I have a co-pay?

  2. This will depend on your insurance policy.  The co-pay amount is listed on most insurance cards.   Co-Pays will be collected at the time of your visit.
