Your Visits

Please arrive at least 15 minutes prior to your appointment and bring:

• Identification (government-issued, such as a driver’s license)
• Mammograms/Ultra Sounds/MRI
  • Bring Film or CD of Images and Reports
• New Patient forms
• Insurance card
• Co-pay as required by your insurance company
• Referral form from your primary care doctor if required by your insurance

**Please note: If any of the above items are missing, your appointment may be rescheduled

New Patients can save time by completing Registration Form and the Radiology Request Form in advance.  This can be downloaded from our website.
